I used many of the ideas you wrote about and lost 50 pounds between April, 2008-March, 2009, so everything that you wrote is very true and very very helpful. It's almost a year later and I've kept off 45 of them, and the other 5 will fall away soon (too much eating out these past couple of months!). When was the last time you had your body fat percentage checked?  Make a habit of doing this once a week to check your progress.  If your body is shrinking (lost fat and inches), you are on the way to building your lean body!  If you have been doing the same workout for months, you need a change to break through your plateau. If your workouts are 2 hours long, you are spending too much time in the gym.

And, spot reduction won't work.  Burn total body fat by doing full body strength training and eating healthy food.  This will help you burn fat off your waist and total body.  Doing 1000 ab crunches a day won't get the job done!
